You also confirm that the information provided in this form is true, correct and within your personal knowledge, and you are authorised to submit this request. When we have reasonable doubts concerning your identity, we may ask you to provide additional information necessary to confirm your identity before responding to your requests. You understand that it may take up to one month from receipt of this form before a reply to your request is provided to you. You also acknowledge that the limit may be extended by a further 1 month where necessary, taking into account the complexity and number of any requests for your personal data.
